The Community Living Program provides a community based setting in which individuals can demonstrate the skills they have gained in an independent environment, while still receiving supervision.  Therapy is taken into the community where skills can be integrated into a daily routine.

The ultimate goal is the complete transition back into the community.  Brookhaven tailors transitional and supported living environments to meet the individual's needs.

Our Transitional Living Center opened in 1991 as the first Oklahoma residential transitional living center for individuals with Traumatic Brain Injury.

Services are presented in "real life" environments to promote compensatory strategies and the transfer of skills.  Staff are available around the clock to assist patients in this process.

Admission criteria:

  • Persons age 18 and older with a primary diagnosis of Traumatic Brain Injury or acquired neurologic problem.

  • Individuals must be functionally mobile, with or without assistive devices.

  • Individuals must be capable of feeding themselves with minimal assistance.

  • Individuals must be continent or approaching continence.
